Documents we need from you before your trip!

WITHIN 30 DAYS PRIOR TO YOUR TRIP

Online passenger information and liability release

All passenger information and liability releases are done online. The more information we have about you (including dietary restrictions or preferences), the better that we can serve you.

Please complete your passenger document/liability release no later than 30 days prior to your departure date.

Before you start, gather all pertinent documents such as dive certifications, medical and dive insurance information, etc. Please read the instructions. Once you start, you will have a 4-hour window to complete the form otherwise you will need to restart the process. If you have any difficulty with the form, it is typically due to the date format. We use MM/DD/YYYY. Therefore, January 17, 2023, would be 01/17/2023.

NEW FOR RETURNING GUESTS! We have a portal for returning guests, so you do not have to re-enter your information. Please click below and a message will be sent to Valerie in Guest Services. She will send you your own link and password to the Customer Portal Login.

Complete the online COVID-19 Waiver

You should have already sent us a photocopy of your COVID vaccination card. If you do not have one, we require proof of a negative COVID test taken within 72 hours prior to boarding. Any test is fine as long as it provides a document with your name on it. Prior to boarding, email this to Documents@MexicoLiveaboards.com.
