Online passenger information and liability release
All passenger information and liability releases are done online. The more information we have about you (including dietary restrictions or preferences), the better that we can serve you.
Please complete your passenger document/liability release no later than 30 days prior to your departure date.
Before you start, gather all pertinent documents such as dive certifications, medical and dive insurance information, etc. Please read the instructions. Once you start, you will have a 4-hour window to complete the form otherwise you will need to restart the process. If you do not have the travel information at this time, you can update this later using this link:
Online Passenger Travel Update
If you have any difficulty with the form, it is typically due to the date format. We use MM/DD/YYYY. Therefore, January 17, 2023, would be 01/17/2023.
NEW FOR RETURNING GUESTS! We have a portal for returning guests, so you do not have to re-enter your information. Please click below and a message will be sent to Valerie in Guest Services. She will send you your own link and password to the Customer Portal Login.
Once you complete this information, you can download and print a copy for your records.
